Log into your EMERGE account. Once logged in, you will see the forms that your healthcare provider has requested you to complete.

Next, you're going to select the form that you need to complete.

From here, you're going to answer all of the questions for questions that have the asterisks. Next to them, you need to complete those to complete the form.
4. Reviewing the Form
Before submitting your form, please ensure that you have accurately answered all the questions. You will not be able to edit the form after submission.
5. Submitting Your Completed Form
Once you have finished, you can either click "Save as Draft" to clear all entries in the form, or select "Submit Form." When you have completed the form, choose "Submit Form."
