Linking Custom Form Fields to the CPP in EMERGE

This tutorial shows you how to link intake form fields to the patient's CPP in EMERGE. You will learn to create and configure forms to ensure patient data is properly saved.

1. Select Custom Forms

To link a specific intake field to the CPP, the first step is to select "Custom Forms".

2. Edit a Current Form or Create a New Form

From this point, you can either link the CPP to an existing form or create a new one. For today's tutorial, let's create a new form.

3. Select the "New Form" Button

Click the "New Form" button to start creating a new intake form.

4. New Custom Form: Title

In the "Title" field, type the name of your custom form.

5. Add a Form Description

Select the "Description" field to enter a description for your form.

6. Select the "Add Field" Button

After writing the title and adding a description, the next step is to click on the "Add Field" button. Next, select the appropriate template for your form and enter the "Field title".

7. Link Field to CPP

Click "Link to CPP" and from the drop-down menu, select the CPP field you wish to link this field to.

8. Save the Custom Form

Once you have completed your custom form, click the "Save" button.


Congratulations! You have successfully linked your custom form field to the CPP.
