Access Patient Intake Forms And Appointment Details

This tutorial explains how to access your intake form, appointment details, and shared documents as a patient through your browser.

1. Select Patient Role

You will learn how to navigate the patient portal to complete forms, view appointment information, and access important documents. Click the option labeled "person I am a Patient" to identify yourself as the patient accessing the portal.

2. Click Login Button

Please enter your email and password, then click the "Login" button to proceed to the patient sign-in page and access your account.

3. Navigating the main screen

On the main screen, you will find all the form requests sent by your provider, along with any future appointments that have been booked.

4. Open Intake Form

Click the "Fill Form" button to begin completing the required intake form provided by your healthcare provider.

5. Review Form Options

You can begin filling out the form and save it as a draft to complete later. Alternatively, if you provide all the required information, you can submit the form.

Please note that once submitted, you cannot modify the information. All forms must be completed prior to the appointment.

6. Confirm Appointment Scheduling

Click the "Confirm Scheduling" button to confirm your appointment with your healthcare provider.

7. View Appointment Details

Click "View details" to see the full information about your scheduled appointment.

8. Prepare For Video Call

If you have a virtual appointment, instructions will be provided here. Please note that you can join the virtual waiting room only 5 minutes before your scheduled appointment time. A button to join the appointment will appear on your home screen.

9. Access Shared Documents

To access shared documents, click the "My Care" section to view your personal health information and shared documents.

10. Open Shared Document

Click the document shared by your healthcare provider, such as "Ontario_Laboratory_Requisition.pdf", to view important lab requisition details for your care.

You have successfully accessed your patient intake form, reviewed appointment details, and opened shared documents through the browser portal. For further assistance, explore additional features in the patient portal or contact your healthcare provider directly.
