1. Introduction
You will complete all necessary steps to register and access your patient profile.
2. Navigate to Registration Page
Open the homepage of the EMERGE Healthcare website to begin the patient account creation process.
3. Select Patient Role
Select the option indicating you are a patient to proceed with the appropriate registration flow.
4. Click Sign Up Button
Click the Sign Up button to start the account registration process.
5. Access Email Entry Field
Click the field to enter your email address, which will be used for account verification and communication.
If you have been invited by a healthcare provider to create your account, please ensure that you use the same email address they have been using to communicate with you.
6. Proceed to Next Field
Please accept the terms of service and privacy policy.
7. Advance to Next Step
Click to proceed to the following section of the registration process.
8. Open Temporary Email Service
Please visit your email service website to retrieve your confirmation email.
9. Confirm Your Account
Click the confirmation link in the email to verify your new patient account.
10. Continue Registration
Click Continue your registration to proceed with entering your personal details.
11. Select Full Name Field
Click the Full Name field to enter your complete name as part of your profile.
12. Select Phone Number Field
Click the Phone Number field to provide your contact number.
13. Select Date of Birth Field
Click the Date of Birth field to specify your birthdate for identification.
14. Open Date Picker
You can also click to open the date picker for selecting your birth year and choose your date of birth.
15. Choose Birth Year
Select your birth year from the available options.
16. Enter Sex Assigned at Birth
Please provide your sex assigned at birth to finalize your personal profile information.
17. Click Continue Button
Click Continue to proceed to the next section of the registration form.
18. Select Address Field
Click the Address field to enter your residential address.
19. Click Continue to Proceed
Click Continue to move forward in the registration process.
20. Choose your province
Please select your province from the list.
21. Click Continue Button
Click Continue to proceed to the following registration step.
22. Select Health Card Type
Click to choose the Health card type
23. Select Health Card Number Field
Click the Health card number field to enter your card number.
24. Select Expiration Date Field
Click the expiration date field to specify your health card's expiry.
25. Select Preferred Pharmacy Field
Click on the preferred pharmacy field to select your pharmacy. You can search by name or address.
26. Click Continue Button
Click Continue to move forward after selecting your preferred pharmacy.
27. Enter Your Password
As a final step, you will need to create a password for your account. Please note that the password must contain at least 8 characters. Ensure you store this password securely for future access.
28. Click to Proceed
Click to continue to the next step in the registration process.
29. Click Explore EMERGE
Click Explore EMERGE! to access your new patient account dashboard and begin using the platform.
You have successfully created a patient account through the browser by completing all required registration steps. You can now access your patient profile and explore available healthcare services on the EMERGE platform.
